Peanut Allergy: Changes in Dogma and Past, Present, and Future Directions. 03.09.2018

Peanut Allergy: Changes in Dogma and Past, Present, and Future Directions. Authors: Sarah Boudreau-Romano, MD , is an Assistant Professor of Pediatrics. Nashmia Qamar, DO, MSci , is an Assistant Professor of Pediatrics. Both authors are affiliated with the Division of Allergy and Immunology, Department of Pediatrics, Ann & Robert H. Lurie Children’s Hospital of Chicago. Abstract: The prevalence of food allergy in the pediatric population, specifically to peanuts, has been rising. Accidental exposure to peanuts in a person who is allergic may have life-threat- ening consequences. Previous recommendations regarding peanut allergy included a de- lay in introduction of peanut to infants. However, more recent studies have provided suf- ficient contrary evidence supporting early introduction of peanuts for prevention of peanut allergy. Therefore, prompt evaluation by a specialist should be considered in infants at high risk of developing peanut allergy. Current treatment is st...

Debates in Allergy Medicine: Allergy skin testing cannot be replaced by molecular diagnosis in the near future Autor:  Désirée Larenas-Linnemann , Investigational Unit, Hospital Médica Sur, Torre 2, consultorio 602, Puente de Piedra 150, Col. Abstract: Percutaneous skin prick tests (SPT) have been considered the preferred method for confirming IgE-mediated sensitization. This reliable and minimally invasive technique correlates with in vivo challenges, has good reproducibility, is easily quantified, and allows analyzing multiple allergens simultaneously. Potent extracts and a proficient tester improve its accuracy. Molecular-bas...

Drug Hypersensitivity and Anaphylaxis in Cancer and Chronic Inflammatory Diseases: The Role of Desensitizations. 31.08.2018

Drug Hypersensitivity and Anaphylaxis in Cancer and Chronic Inflammatory Diseases: The Role of Desensitizations Autor:  Mariana Castells, Allergy and Immunology, Brigham and Women’s Hospital, Harvard Medical School, Boston, MA, United States. Abstract:  Drug allergy is a rising problem in the twenty-first century which affects all populations and races, children, and adults, and for which the recognition, diagnosis, management, and treatment is still not well standardized. Classical and new chemotherapy drugs, monoclonal antibodies (MoAbs), and small molecules to treat cancer and chronic inflammatory diseases are aimed at improving quality of life and life expectancy of patients, but an increasing number of reactions including anaphylaxis precludes their use in targeted populations. Women are more affected by drug allergy and up to 27% of women with ovarian and breast cancer develop carboplatin allergy after multiple cycles of treatment. Carriers of BRCA genes ...

Acquired C1 Inhibitor Deficiency. Autores:   Iris M. Otani, MD ; Department of Medicine, Division of Pulmonary, Allergy, Critical Care, and Sleep Medicine, UCSF Medical Center, 400 Parnassus Avenue, Box 0359, San Francisco, CA 94143, USA. Aleena Banerji, MD ; Department of Medicine, Division of Rheumatology, Allergy and Immunology, Massachusetts General Hospital, Cox 201 Allergy Associates, Boston, MA 02114, USA. Abstract: Acquired angioedema due to C1-INH deficiency (C1-INH-AAE) can occur when there are acquired (not inherited) deficiencies of C1-INH. A quantitative or functional C1-INH deficiency with negative family history and low C1q is diagnostic of C1-INH-AAE. The most common conditions associated with C1-INH-AAE are autoimmunity and B-cell lymphoproliferative disorders. A diagnosis of C1-INH-AAE can precede a diagnosis of lymphoproliferative disease and confers an increased risk for developing non-Hodgkin lymphoma. Treatment focuses on symptom control wit...
